Healthy Lifestyle Support Program

Need help with accountability? Looking to lose some weight? Are you at risk for developing Diabetes? We have partnered with the CDC to offer a group-based program that has been proven to help participants lose weight, adopt better eating habits, and achieve the recommended amount of physical activity. We utilize the Prevent T2 curriculum developed for the National Diabetes Prevention Program. The program consists of 24 meetings, covering different wellness topics, and is led by our certified lifestyle coaches. The program is covered by Medicare for qualified individuals.

Success Stories

Here’s how others have been helped by acac’s Health Lifestyle Support Program!

“There’s no meds at all. It’s all about a lifestyle change. So far, I’ve lost 19lbs. I would say give it a whirl. I would sit down with [someone considering this program] and tell them how much of a change this has made for me.” – Marion Brown, participant

“Since getting married, I had gained 45 pounds. Three years ago I was determined to lose weight and be more active. I had edged toward my goals, but on my own, I had gotten stuck and stymied. I couldn’t get more weight off. With support from the acac coaches and my group, I came to recognize and master small habits one-by-one, week-by-week. By changing my lifestyle, I am now where I want to be and my blood sugar is back in the normal range. I am light on my feet and dancing again. I feel great. Now, I’m even getting certified to be a coach myself so I can help others as acac has helped me.” – Garnett Mellen, instructor & former participant
